Transgender Surgery 2018: Revisiting an Old Paradigm – Summary:

Larry I. Lipshultz, MD, provides an up-to-date definition of gender dysphoria and criteria for urologic transgender surgery. He also shares an interview with a patient prior to surgery and a review, in movie format, of an actual operative procedure.

Abstract:

This presentation focuses on up-to-date diagnostic criteria for gender dysphoria and the treatment of patients consenting to undergo bottom transition surgery. Currently, gender dysphoria impacts about 1.4 million Americans. This presentation reviews the University of Texas/Baylor College of Medicine experience from 1975 to 1995. During that time, this institution provided care to 40 transgender women. An interview with a patient prior to surgery reveals the importance of bottom transition surgery for certain patients. A review, in movie format, then illustrates an actual operative procedure.

This lecture also discusses the effect of estrogen therapy on patients, criteria for surgery clearance, postoperative care, and adverse complications following surgery. Future research should address unmet needs in this area, such as long-term medical and mental health outcomes, ultimate comorbid conditions, and adjuvant transgender interventions, such as speech therapy and appearance coaching.

Larry I. Lipshultz, MD

Larry I. Lipshultz, MD, is Professor of Urology and Chief of the Scott Department of Urology’s Division of Male Reproductive Medicine and Surgery at Baylor College of Medicine in Houston, Texas. Dr. Lipshultz, who holds the Smith Chair in Reproductive Medicine, is a well-known authority on abnormalities of male reproduction, erectile dysfunction, and male hormone therapy. He has published more than 390 journal articles and has instituted a fellowship training program in male reproductive medicine and surgery that has trained more than 80 physicians who are now in practice both here and abroad. Dr. Lipshultz received his MD from the University of Pennsylvania in Philadelphia and completed a fellowship at the University of Texas at Houston as the first AUA research scholar. His professional interests include vasectomies and their reversal, male infertility, in vitro fertilization, erectile dysfunction, low testosterone, Peyronie’s disease, and sexual wellness. He is a member of numerous medical organizations, including the American Society for Reproductive Medicine (of which he is also a former president), the American Society of Andrology, the Houston Reproductive Society, and the Houston Urological Society.
